
Switching a machine off does not make it safe. Every year, equipment starts up while a maintenance worker, an operator, or a cleaner still has hands inside it. The results are amputations, crush injuries, and electrocutions. In nearly every one of those cases, someone believed the machine was dead. Stored pressure, a residual charge, a coworker who knew nothing about the repair, or a switch that nobody locked usually proves them wrong.
The following covers what the energy control rules require and where they break down. It also explains what an injured Illinois worker can pursue afterward.
Stored Energy Does Not Disappear When the Power Goes Off
Hazardous energy is not only electrical. A press holds hydraulic pressure long after the pump stops. A conveyor on an incline rolls backward under its own weight. Capacitors hold a charge, springs hold tension, and steam lines hold heat. Pneumatic systems hold air that will drive a cylinder the instant someone bumps a valve. Flipping a control switch stops the machine from running. It does not release what the machine still holds.
Energy control therefore works as a procedure rather than a single action. The worker isolates the equipment at every energy source. The worker then locks each isolating device in place. Only after that does anyone relieve, block, or bleed off the stored energy. Crews skip that last step constantly, and it shows up again and again in serious maintenance injuries. The machine looks completely inert right up until it moves.
What OSHA Requires Before Anyone Services a Machine
The governing rule for general industry is 29 CFR 1910.147, the control of hazardous energy standard. It demands three things from every employer. A written energy control program. Training for the employees who will use it. Periodic inspections to confirm the crew actually follows the procedure. Before servicing begins on equipment that could start up or release stored energy, the employer must isolate that machine and render it inoperative.
The standard also states a clear preference. Lockout wins whenever an isolating device can accept a lock. Tagout works only when the device cannot take a lock, or when the employer can show that tags alone fully protect the worker. A tag sends a message. A lock creates a barrier. For any equipment installed or overhauled after January 2, 1990, the isolating devices must accept a lock in the first place.

The Steps a Proper Lockout Follows
A compliant procedure is short. Each step exists because a crew once left it out and someone paid for it. The authorized employee identifies every energy source feeding the machine, not just the obvious one. That employee then warns everyone the shutdown affects. The crew stops the equipment through its normal shutdown sequence. Each isolating device moves to the isolated position. Every authorized employee applies a personal lock, which rules out shared locks and rules out a supervisor locking on someone else’s behalf. The crew then relieves or restrains whatever energy the machine still stores.
One step remains, and crews rush it more than any other. Someone tries to start the machine through its normal controls to verify the isolation, then returns those controls to off. That check takes seconds. It is also the only step that tests whether the rest of the sequence worked.

Where Energy Control Programs Break Down
Written programs are common. Programs that match the equipment on the floor are less common. Plants modify machines, add a second feed, or buy used equipment with different isolation points. The laminated card on the wall then describes a machine that no longer exists.
Production pressure supplies the rest of the failures. A crew treats a jam as a quick fix rather than servicing, so nobody locks anything. A supervisor sends a temporary worker to clean equipment and never trains that worker as an authorized employee. One lock stands in for a six person crew, so a coworker releases the machine while someone else is still inside it. Contractors and host employers each assume the other one runs the procedure. Shift change lands in the middle of a repair, and the incoming crew inherits an unlocked machine.
Guarding and Lockout Are Two Halves of the Same Problem
During normal production, guards do the work. OSHA’s general machine guarding requirement obligates employers to guard the point of operation, ingoing nip points, rotating parts, and flying chips and sparks. No exemption exists for old equipment, and none exists for machines a company inherited along with a building.
The energy control rules take over the moment a worker opens or bypasses that guard. Investigators often find both failures sitting together. A guard vanished months ago, on a machine nobody locks out either. A missing guard also raises a question about the equipment itself. Some machines reach the floor without adequate interlocks or with defective safety devices. Those facts can support a product liability claim against the manufacturer, entirely separate from anything the employer did.
Workers’ Compensation May Not Be the Whole Claim
Illinois workers’ compensation covers a job injury regardless of fault, and it ordinarily blocks a direct negligence suit against the employer. Benefits reach medical care, temporary disability payments during time away from work, and permanent disability compensation. The Illinois Workers’ Compensation Commission handbook lays out how each category works.
What that system does not do is reach everyone who contributed to the injury. Machine cases often involve parties well outside the employer. The equipment manufacturer may share blame. So may a maintenance contractor, a general contractor running the site, or the client company that borrowed a worker from a staffing agency. So may a vendor who modified the machine and pulled an interlock to speed up throughput. Claims against those parties run on their own track, and the benefit schedule does not cap them. Anyone hurt in a startup incident should understand both routes before signing anything.
Evidence That Disappears After a Machine Injury
Repairs follow a serious incident within days. Guards go back on, procedures change, and the machine soon stops resembling the one that caused the injury. Photographs of the equipment in its condition right after the incident matter enormously. So do the written energy control procedure for that specific machine, the training records for everyone on the crew, the maintenance logs, and the names of coworkers who watched the sequence unfold.
